If it is not in "ready" mode, could there be a problem with the brake
switch? Are you certain that the brake was depressed when you turned it on?

Definitely sure. But as it seems to happen one time in say 20, Im wondering
if it is some minor fix needed.

When this problem was first mentioned I wrote that it didn't occur in my
2005. That may not be the case. As the Prius starting procedure became
routine I performed it much more quickly. There have been a few occasions
where I caused the symptoms you experience by not hitting the brake pedal at
the precise moment the power button was depressed. I correct my error by
holding my foot on the brake and hitting the power button again. Could you
be doing the same thing?

I'd say that if the READY light isn't on, you didn't have your foot
firmly on the brake when you turned the Prius on. All you should need
to do is make sure you got that brake, and hit the switch again.
