 Usually power conditioners monitor the incoming frequency and voltage and
(usually) maintain a much more precise frequency whether it be 60 Hz or 50
Hz depending on which country you are in.  It also maintains a steady
voltage level by clamping down on voltage fluctuations that occur during
different times of the day.  They also can filter out any Radio Frequency
Interference that may be present.  Some models have a battery that will
briefly keep the voltage level at a preset level in the event of a "brown
out".

Basically, they keep your incoming power nice and "clean" and your
electronics will thank you for it.  The closest analogy I can come up with
is to liken them as the fuel filter in your car - chances are the fuel is
already clean at the gas station, but the filter is there to ensure it stays
clean prior to consumption.
